The Provider Application must be completed only once during the period from June 1 through May 31 each year. The provider application gives vital organizational information and assures that each CEU provider will abide by the appropriate guidelines in connection with the CEU process. There is no fee required for submitting the Provider Application.
The Program Application must be submitted at least 30 days prior to the initial offering date of the program. This requirement can be waived upon request if extenuating circumstances are evident. Once approval is granted, this office must be notified in writing of any subsequent offerings of the approved program, date, time and place. Approval for each program is good for one year from the date of approval.
There is no fee for applying for the approval, however there is a fee for each participant who requires that a certificate be issued. The fee is on a sliding scale basis and is determined by the number of CEUs awarded, i.e. $10 for .5 to 1 CEUs, $15 for 1.1 to 1.5 CEUs. etc. Please note that 60 minutes equals one contact hour and one contact hour equals .1 CEU. We cannot approve a program which is less than five contact hours in length.
There are three specific methods of handling the certificate process and it is enumerated on page two of the Program Application. Providers are asked to evaluate their requirements and determine the best method for their organization for this process.
In order to obtain a transcript of all CEUs earned after June 1, 2000, or if you need a copy of a specific CEU certificate, please fill out the Transcript Request Form and mail it to:
