Student Conduct Pre-Conference Information Form
Mississippi College students must fill out the following form within 24 hours of being issued a "Student Conduct Summons." A student who has been issued a "Student Conduct Summons" has typically been involved in an incident which may have included violations of the Mississippi College Student Code of Conduct. "Student Conduct Summons" are issued by Mississippi College officials, including Residence Life staff professionals and paraprofessionals (Resident Assistants), Student Life professionals, and Office of Public Safety officials.
This form should be filled out in its entirety. The purpose of the following questions is to collect as much information as possible about the alleged incident. There are five sections to this form which can be accessed by clicking “Next Section” or “Previous Section” at the bottom of each page. The sections are:
- GENERAL INFORMATION
- YOUR RIGHTS AND PRIVILEGES AS A STUDENT IN A STUDENT CONDUCT PROCEEDING
- STATEMENT OF UNDERSTANDING
- INFORMATION ABOUT THE EVENT IN WHICH YOU WERE ALLEGEDLY INVOLVED
- ADMINISTRATIVE HEARING CONFERENCE INFORMATION
You will be contacted via campus email within three school days of filling out this form about any charges regarding violations of the Mississippi College Student Code of Conduct and/or setting up a time for an Administrative Hearing Conference. Therefore, it is very important that you check your campus email DAILY.
